Special Support Program Application Form Side A
If you grant consent for the Ministry of Health to access your income information through CRA, please fill out Side A form. No annual application required.
Special Support Program Application Form Side B
If you do not grant consent for the Ministry of Health to access your income information through CRA, fill out Side B form. You must apply for coverage every year under this option.
